Before deciding to invest in funds managed by TMBAM, please examine the funds prospectus carefully. The prospectus can be obtained by contacting TMBAM, its branch offices, or any of our selling agents. To open a fund account for the first time, please fill out the following applications and submit them in person at TMBAM, its branch offices, or any of our selling agents.

Application Forms for New Accounts
  1. New Account Opening
    • Opening a new account at TMBAM or its branch offices
    • Opening a new account at TMB Bank or a selling agent
  2. Direct Debit Form. Unitholders may arrange to automatically debit their bank accounts with TMB, BBL, KBANK, KTB, SCB, BAY, UOB or TBANK when they purchase unit trusts. To designate more than one bank account, please fill separate forms for each bank account. Processing normally takes up to 3 weeks; TMBAM will subsequently notify the unitholder after the host bank has permitted the direct debit arrangement.
  3. Internet Application Form. Get the ultimate in convenience by going on-line. www.tmbam.com lets you devise an investment plan, buy, sell, or switch unit trusts yourself. Keep track of NAVs of all funds, your portfolio, see transaction histories, and a host of other functions. Those wishing to transact on line should have a direct debit with a bank account approved by the host bank before placing orders.
Documents Required for Account Opening
Those applying for an account must submit the following required documents described below and provide copies of bank accounts that they wish to use in conjunction with the fund account.

Type of Investor Required Documents
  • Individuals
  • Copy of official ID, civil servant ID, state-enterprise ID, or passport
  • Copy of house registration (if identification does not indicate address of residence)
  • Foreigners are required to submit copy of Work Permit or Visa valid for at least another 3 months
  • Shopowners
  • Copy of official ID, civil servant ID, state-enterprise ID, or passport
  • Copy of house registration (if identification does not indicate address of residence)
  • Copy of business registration showing name of applicant
  • Copy of VAT registration (if any)
  • Limited Partnerships or General Partnership
  • Copy of official ID, civil servant ID, state-enterprise ID, or passport of authorized individuals and major shareholders holding 30% or more
  • Copy of house registration of individuals authorized to make payments and major shareholders holding 30% or more (if identification does not indicate address of residence)
  • Copy of partnership registration certifying list of managing partners and scope of duties of managing partners
  • Copy of partnership agreement or articles of association
  • Copy of tax registration
  • Company Limited
  • Copy of company affidavit
  • Copy of shareholder registration
  • Board of Director resolution for the company to open the account or conduct business transactions
  • Power of Attorney document (if any)
  • Copy of official ID, civil servant ID, state-enterprise ID, or passport of the following individuals
    • Shareholders holding 30% or more
    • Two authorized directors (one managing director and one authorized director)
  • Copy of house registration of authorized directors (if identification does not indicate address of residence)
  • Copy of VAT registration
  • Copy of Memorandum of Association
  • Copy of tax registration
  • Associations and Charities
  • Copy of license for establishment
  • Copy of Articles of Association
  • Copy of business registration (if any)
  • Resolution from the Board Committee to open the account or conduct business transactions
  • Copy of official ID, civil servant ID, state-enterprise ID, or passport of authorized individuals
  • Copy of house registration of authorized individuals (if identification does not indicate address of residence)
  • Document showing tax-exempt status (if any)
  • Temples
  • Copy of temple condition certification
  • Copy of official appointment of abbot
  • Copy of buddhist monk identification issued to monks and novices
  • Schools
  • Copy of license from the Ministry of Education or Ministry of University Affairs
  • Resolution from the Board Committee to open the account or conduct business transactions
  • Copy of official ID, civil servant ID, state-enterprise ID, or passport of authorized individuals
  • Copy of house registration of authorized individuals (if identification does not indicate address of residence)
  • Diplomatic Missions
  • Official letter instructing to open of the account or conduct business transactions
  • Passport of individuals which are authorized to act on behalf of the diplomatic mission
  • List of authorized individuals
  • Non-Resident Accounts (Juristic Persons)
  • Resolution of the Board
  • Copy of registration of juristic person
  • Copy of business registration certificate
  • Copy of official ID or passport of management staff
  • Copy of Memorandum of Association
  • List of management executives
  • Government Offices, Government Agencies, State Enterprises
  • Copy of legal act to establish the entity
  • Power of Attorney to instruct the account opening
  • Copy of official ID, civil servant ID, state-enterprise ID, or passport of authorized individuals
  • Copy of house registration of authorized individuals (if identification does not indicate address of residence)
  • Instructions to open the account, designation of authorized individuals to act on behalf of the entity, and conditions for payment.
  • Collective Investment Schemes (i.e.foreign registered funds)
  • Copy of establishment of the CIS
  • Copy of the business registration of the CIS
  • Document evidence confirming that the CIS operator is a licensed asset management company.
  • Copy of official ID, photo ID issued by a government agency, or passport (if foreigner) of authorized individual of the CIS and document certifying that this authorized individual is empowered to act on behalf of the entity.

Notes:
  1. Please sign all copies to endorse the validity of all personal document and evidences accompanying the application.
  2. xxxx of juristic persons must be endorsed by the Registrar within a period not exceeding one month till the date the application is submitted or the applicant's identity is being established.
  3. Copy of bank account for direct deposit of proceeds from the sale of unit trusts, if desired. Please sign all copies to endorse the validity of the document.
Materials Received After Opening of Account
  1. Copy of account application and of other services applied
  2. TMBAM Card and PIN code for use in placing orders through the following channels:
    • The 24-hour automated telephone system (IVR)
    • ATM machines of TMB Bank
    • Customer Service Center Tel. 1725
Recommendations by TMBAM
  1. The name of the account must be identical to the name shown on the bank account, to prevent situations where commercial banks could refuse to transfer payment of sales proceeds into the account.
  2. Applicant's TMBAM accounts can be linked to their TMB Bank accounts to facilitate deductions for unit trust purchases or have sales proceeds deposited automatically into their bank accounts.
  3. Unitholders residing in upcountry provinces (outside of Greater Bangkok) should designate to have sales proceeds deposited into a TMB Bank account.
